01Acceptance of These Terms
These Terms of Service (the “Terms”) govern your use of the Reflect Wellness website and the online booking and intake tools available through it (together, the “Site”). By using the Site, you agree to these Terms. If you do not agree, please do not use the Site.
These Terms cover the Site only. Your medical care is governed by separate consents and agreements you sign with your treating clinic or provider, and those documents control with respect to your care.
02Eligibility
The Site is intended for adults. You must be at least 18 years old to use the Site, book services, or submit intake information. By using the Site you represent that you are 18 or older.
03Description of Services
Reflect Wellness operates wellness clinics in Murfreesboro and Franklin, Tennessee, offers telehealth visits to Tennessee patients, and offers a national telehealth program for compounded medications.
Tennessee clinics and telehealth. Services at our Tennessee locations and through our Tennessee telehealth visits are provided by the licensed providers of our practice.
National telehealth program. On the national program, medical assessments are conducted by an independent network of licensed providers made available through a partner telehealth platform, not by Reflect Wellness. Reflect Wellness does not prescribe medication on the national program and does not control the independent providers' medical judgment. Whether any medication is appropriate for you, and whether a prescription is issued, is decided solely by the licensed provider who evaluates you. Completing an assessment does not guarantee that a prescription will be issued. If a prescription is issued, it is filled by a licensed 503A compounding pharmacy selected through the provider network.
Availability of specific programs and medications varies by state.
04Website Content Is Not Medical Advice
Content on the Site, including pages describing treatments and programs, is provided for general information only. It is not medical advice, does not create a provider-patient relationship, and is not a substitute for evaluation by a licensed provider. Do not rely on Site content to diagnose or treat any condition. Always consult a licensed provider about your individual situation.
05Medical Emergencies
The Site is not for emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. Forms, messages, and booking tools on the Site are not monitored for urgent medical needs.
06Accounts, Booking, and Communications
When you book a visit or begin an intake through the Site, you provide contact information such as your name, phone number, and email address, and for the national program additional intake details. You agree to provide accurate, current information and to update it if it changes.
Text messages and calls. If you provide your phone number and consent, we may contact you by call and text message, including by automated technology, about your bookings, your programs, and our services. Consent to marketing texts is not a condition of purchase. Message and data rates may apply and message frequency varies. You can reply STOP to opt out of texts or HELP for help at any time. The full terms of our SMS program, including how to opt out of each channel, are described in our Privacy Policy.
Email. If you provide your email address, we may send you service and marketing emails. You can unsubscribe from marketing emails at any time using the link in the email.
07Payments and Refunds
Prices for services and programs are presented at booking or checkout. Payments for the national telehealth program are processed by a secure third-party payment processor. Payments for in-clinic services are processed through our in-clinic systems. We do not store your full payment card details on the Site.

08Prohibited Uses
You agree not to:
- Use the Site for any unlawful purpose or in violation of these Terms.
- Submit false, misleading, or another person's information in a booking or intake form.
- Attempt to gain unauthorized access to the Site, its systems, or other users' information.
- Interfere with the operation of the Site, including by introducing malicious code or placing unreasonable load on our infrastructure.
- Scrape, harvest, or collect information from the Site by automated means without our written permission.
- Copy, resell, or commercially exploit Site content except as allowed by Section 9.
09Intellectual Property
The Site and its content, including text, graphics, logos, images, and software, are owned by Reflect Wellness or its licensors and are protected by intellectual property laws. We grant you a limited, revocable, non-exclusive license to access and use the Site for your personal, non-commercial use. No other rights are granted. “Reflect Wellness” and our logos may not be used without our prior written permission.
10Disclaimers and Limitation of Liability
The Site is provided on an “as is” and “as available” basis. To the fullest extent permitted by law, Reflect Wellness disclaims all warranties regarding the Site, express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or secure.
Nothing in this section limits or modifies the duties any licensed provider owes you in connection with medical care actually provided to you. This section addresses your use of the Site.
To the fullest extent permitted by law: (a) Reflect Wellness will not be liable for indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of the Site; and (b) Reflect Wellness's total liability arising out of or relating to the Site will not exceed the greater of one hundred dollars ($100) or the amount you paid us through the Site in the twelve months before the claim arose. Some jurisdictions do not allow certain limitations of liability, so some of these limitations may not apply to you.
11Indemnification
You agree to indemnify and hold harmless Reflect Wellness and its owners, employees, and agents from claims, damages, and expenses (including reasonable attorneys' fees) arising out of your violation of these Terms or your misuse of the Site, to the extent permitted by law.
12Governing Law
These Terms are governed by the laws of the State of Tennessee, without regard to its conflict of laws rules. Any dispute arising out of these Terms or your use of the Site will be brought in the state or federal courts located in Tennessee, and you consent to their jurisdiction, except where applicable law gives you the right to proceed elsewhere.
13Changes to These Terms
We may update these Terms from time to time, including after attorney review of this draft. When we do, we will post the updated Terms on this page and update the effective date. Your continued use of the Site after changes are posted means you accept the updated Terms.
